DIRECTION FOR COMPLETING SURVEY
The Cardiopulmonary Science program at Florida A&M University is collecting the employer
surveys on our graduates to complete our annual report for accreditation of our education
program. The survey you will be using is a national survey generated by the Commission
on Accreditation for Respiratory Care Programs. The purpose of the survey is to evaluate
the effectiveness of the cardiopulmonary science program. The system you will be using
in helping us to complete this task is a database service provider that we use to
collect the data and help generate the necessary reports. If you would please follow
the directions below and complete the survey in a timely fashion we would greatly
appreciate it. Thank you for your help in this matter.
We are asking you to complete an on-line survey to evaluate specific performance of
individual graduates of our programs who are or were employed by your organization.
Directions: In the web section for Surveys you will need to click on Employer Surveys.
When the log on and password box appears, please enter the following information:
Log in: famucpsemploy (your log in)
Password: famucpsemploy (your password)
This will bring you to the employer survey pages. You will need to complete a survey
for the graduate(s) listed that worked at least six months for you. You will note
that several of the items are required fields and these must be completed before submitting
your survey. You will need to enter the name of individual names of graduates and
year for that cohort of graduates. When you have completed the survey, click the submit
button and you should receive a response page that indicates the survey has been submitted
successfully. All information provided to us will be kept confidential and used for
program evaluation only. The names of graduates and graduation year are posted on
the next page.
